Maple Creek Home Health and Hospice offers hospice services that are covered 100% by Medicare, Medicaid, and many insurance providers. Your doctor DOES NOT have to order hospice services or refer you to hospice for us to come out for an assessment. Give us a call and we will come to you for a hospice assessment at no cost. In the event hospice is the appropriate service, we will contact your doctor to discuss hospice.

Maple Creek Home Health and Hospice takes a holistic approach to care that focuses on all aspects of the patient’s physical, spiritual, and emotional well-being. Our team of hospice experts work together to ensure the needs of the patient are met and the patients’ caregivers and family receive assistance with planning, education, and support necessary to prepare for their loved one’s passing. Hospice care is provided in the patient’s residence which includes a home, assisted living, or skilled nursing facility.

Hospice is a benefit available to those who have a prognosis of 6 months or less. This can be due to a terminal illness, or an overall decline in the person’s health. Hospice is palliative in nature that neither hastens nor postpones death but focuses on the quality of the patients remaining time on earth.
